Problem

Patients stopped logging symptoms after the first week, so doctors saw gaps instead of trends.

01 Context

Pulse helps people with chronic pain keep a diary for their doctor. The old diary had 14 questions, and 70% of new users stopped after week one.

02 Research

I ran a two-week diary study with 9 patients and sat in on 4 appointments. Doctors only looked at three things: pain, energy and sleep.

03 Decisions

We cut the check-in to those three, with big tap targets you can use with a sore hand. Notes became optional. We lost some detail, so the weekly summary asks one follow-up question instead.

04 Result

Weekly check-ins rose 41%, the average check-in takes 38 seconds and doctors now get a one-page summary before each visit.
